Our goals are to fight against care disruptions, to reinforce the dynamics of community care, to improve fluidity between community medicine and the hôpital de Hyères, and to break the feeling of isolation that can sometimes exist among professionals. In short, the CPTS allows healthcare professionals to work under better conditions." Thus, the CPTS aims to boost the attractiveness of the Hyères territory and, thereby, improve the management of unscheduled care.

FIELD SOLUTIONS

He continues: "The CPTS were driven by the Agence Régionale de Santé (ARS) and the Caisse Primaire d'Assurance Maladie du Var (CPAM) to create a link between administrations and healthcare professionals because administrations lacked expertise and solutions from the field. The strength of our CPTS is that it is made up of practicing healthcare professionals, which ensures we have an excellent knowledge of the field. This is essential for understanding the difficulties we can face." In Hyères, the CPTS is chaired by Guillaume Mouchotte, a general practitioner. On a daily basis, he is surrounded Guillaume Mouchotte insists: "We want as doctors to participate in a better quality of life for professionals and patients." Concretely, the members of the CPTS campaign for an improvement in local healthcare pathways. Thus, the general practitioner continues: "Our action is an essential factor for patients with chronic illnesses or losing autonomy, particularly for the elderly population. For the CPTS, the goal is to facilitate access to a primary care physician and to all the professionals needed for a healthcare pathway. We also ensure continuity of care on an outpatient basis, meaning outside of the hospital. Thanks to the CPTS, we have set up a telephone number that allows patients to reach a healthcare professional 7 days a week. Indeed, a telephone on-call system is in place, and we can be reached for any paramedical care requests. We also support users in their search for a primary care physician. Furthermore, our action is essential for unscheduled care and to avoid overloading the hospital with non-urgent emergencies. All doctors who are members of the CPTS offer unscheduled care slots, and almost all doctors participate in the SAS (Service d’Accès aux Soins). This service, reachable by dialing 15, provides medical regulation and directs patients to a general practitioner in the absence of their own. Thus, community medical issues remain in the community, preventing emergency rooms from being overloaded. However, despite their goodwill, the members of the CPTS face unexpected administrative resistance, as Guillaume Mouchotte regrets: "The public-private system remains complicated. The CPTS coordinates private practitioners, and we try to coordinate with public practitioners. But there are still administrative barriers that are difficult to break down"! In addition, the team intervenes on the issue of prevention as part of annual vaccination campaigns, as is the case for flu or the new variant of COVID. "We provide a healthcare and prevention offering. Our prevention actions are essential, for example for the human papillomavirus vaccination campaign. To this end, 2 doctors visit the 3 public collèges of Hyères to explain the reasons for this vaccination and to vaccinate the target audience (5ème students) with parental authorization. Year after year, the vaccination rate is progressing, but we are still far from the goal," notes the general practitioner. However, the benefits of the campaigns are not immediate, and sometimes we only see the effects two or three years later. For her part, Alvina Broc, coordinator of the CPTS, continues: "We also work with diabetics in coordination with the hospital. Thus, patients with a complex file are referred to Dr. Caroline Solmon, a diabetes specialist. She sees patients at the request of the primary care physician within a shortened timeframe, then provides us with the treatment elements we might be lacking. Her intervention allows for the management of our most complex cases, and, in turn, we benefit from her expertise in the subsequent treatment of our patients. This collaboration works perfectly and yields excellent results."•
